CarePlus is a Primary care Initiative funded by the Ministry of Health and is designed to enhance the care of patients who have high needs. With a strong focus on education and preventative care, it aims at managing chronic illnesses such as Asthma, Diabetes, and Obesity more efficiently. Goals are set and a care plan is developed for each patient in a partnership where the patient and health team work together to achieve these goals. The program is available to our enrolled patients, and regular appointments are made with our Practice Nurse to monitor your progress.
